Our team of professional Aging Life Care Managers® and Placement Specialists serve the Denver, Colorado, community. We help families navigate healthcare, manage dementia or Alzheimer’s, mobility issues, disabilities or other challenges coupled with aging or disability. We are consultants that help coordinate options for care and do not provide hands-on care. We do help find hands-on care if needed.

Navigating the transition of an elderly family member into a higher level-of-care community is an emotionally taxing journey. No matter how much it is needed and how much care is given to finding the right option, it is still a heartbreaking process for most families and the stress can be unbearable. It’s a decision often fraught with worry, guilt, and uncertainty. When faced with this challenge, families must carefully consider several factors to ensure their aging loved one’s comfort, safety, and overall well-being.

First and foremost, assessing the level of care needed is crucial. Understanding the specific medical and personal care requirements of your loved one is our focus. Are they dealing with mobility issues, memory loss, chronic conditions, or do they require assistance with daily tasks, or have fall risk concerns? Our placement services involve consultation with an Aging Life Care Manager® or Geriatric Care Manager. This evaluation will help organize their unique care needs and provide a roadmap for the type of care they need for now and into the future. This professional consultation helps our placement consultants narrow down the specific communities that can adequately meet their needs.

Narrowing down the list is just the beginning. Your placement consultant will then consider the amenities and services offered by potential communities along with how they could manage the care needs for your loved one. This means looking beyond the basics of healthcare and focuses on the quality of life your loved one will experience, as well as how the community would meet the wishes of the elderly family member. This includes recreational activities, meal options, socialization opportunities, and any specialized care programs tailored to their interests and needs.

Location is another vital aspect to contemplate. Is it important for your loved one to remain close to family and friends? Are there medical facilities or cultural amenities they should have easy access to? Assessing these factors will help determine the most suitable geographic area for their new home.

Financial considerations cannot be overlooked. Understanding the costs associated with different care communities and whether they align with your loved one’s budget and financial resources is a high priority. Together, we will explore options such as long-term care insurance, government assistance programs, or veterans’ benefits to help alleviate the financial burden and help negotiate if there is room for such.

Due to these complexities, it is very important to work with a placement consultant who provides invaluable support. Our team of placement consultants and professional care managers have the expertise and resources to assess your loved one’s needs comprehensively and match them with suitable care communities. Our placement consultants have extensive knowledge of local facilities such as programs offered along with styles of communication and management. We understand the nuances of different care options and we’ll work with your family through the decision-making process with empathy and understanding.

You will pay for the care management assessment and consultations to create a plan because this is something that you will be able to use whether you move them into a new care facility or continue to manage their care as a family. You will not pay for the placement consultations and our time to take you on tours and answer questions about the various facilities. You also do not pay for any time spent in research or negotiation by our placement consultants. Those fees are covered by the community you choose.

By collaborating with a placement consultant, families alleviate much of the stress and uncertainty associated with finding the right care community for their aging loved one. Together, we’ll explore various options, weigh the pros and cons, and ultimately make a decision that prioritizes the comfort, safety, and happiness of your beloved family member.
